Increased number and increased requirements for reception and patients ready for discharge from hospitals generate the need for more efficient management of patient flow in the municipal health and care services. There are also increased requirements for interaction between the municipality’s care services. Tønsberg municipality therefore wants a system that can improve the interaction in a holistic clinical pathway. Tønsberg municipality wants interaction between the various levels in the municipality and other health care providers that are parts of a clinical pathway for which the current patients’ record system is lacking functions. As well as a system for follow up the clinical pathway internally in the individual service and department.

The needs are demonstrated by i.a.:

– The patients’ status and where they are in the care chain

– Expected entry in and release date from the municipal services

– All participants in the clinical pathway need updated information
